Clients must confirm their ownership of the animal(s) and authorize Purdue University Veterinary Hospital (PUVH) for examinations, treatments, and emergency care. This consent includes all services by veterinarians, nurses, assistants, and students involved in the care.
An upfront payment is required at the Emergency Department check-in, with additional fees applicable if the animal presents in an unstable condition. Authorization is also granted for isolation services in cases of suspected or confirmed infectious diseases.
Client acknowledges that PUVH may use medical data and images of the patient for educational and research purposes as part of its academic function.
Clients accept full financial responsibility for all treatments and must provide deposits for admitted patients based on provided estimates. Regular updates on medical and financial status will be given, especially when patients are hospitalized.
Full payment is due at the time of discharge, with various accepted payment methods including cash, checks, credit/debit cards, and CareCredit. Unpaid balances are subject to additional fees if not settled within specified times.
Outstanding balances are actively pursued and may incur collection fees. Severe delinquencies may lead to collection agency involvement and reporting to credit bureaus.
