Experience Report Instructions

The following information will assist you in completion of this online form. Click here to find out what information you will need to complete the Experience Report.

This Experience Report should be completed by CODO and re-entry and re-entry post-baccalaureate applicants applying to the on-campus Veterinary Technology Program only. Transfer and Regional Campus Transfer applicants will complete the same Experience Report as part of their Admissions application.  If you are not interested in the on-campus Veterinary Technology Program, do not fill out this form. If you are still unsure if you should continue with the Experience Report, click here.

Completing the Experience Report

1. Applicants may fill out the experience report beginning December 15, 2017, visit the online Experience Report via www.vet.purdue.edu/application.

2. If this is your first time at the site, you will need to create a user account.  To create an account, please click here.

a.  For a new account, you MUST select the "Veterinary Technology Experience Report" option from the drop down menu on the create account page.

3.  Once you have created your new account, you should begin to complete the requested information.  You should save your work frequently. You will be allowed to log back into the Experience Report as needed.  Please make note of your username and password as it will be required to log back into the Experience Report.

4.  When you feel that you have completed the entire Experience Report and are ready for your final submission, please select the "final submission" tab and then click on the "submit" button.  You will receive an email stating that your Experience Report has been submitted.
